Navigating HealthBase: Dashboard, Patients, and Reporting
When you log into ITO Health, you’ll see three main navigation tabs at the top of your screen: Dashboard, Patients, and Reporting. Here’s what each tab does and how to use it.
1. Dashboard: Your Daily Hub
The Dashboard is your central hub for staying on top of daily tasks and urgent patient challenges. It gives you a quick snapshot of what needs attention.
| To-Do List | View all tasks assigned to you so nothing falls through the cracks. |
| Urgent Challenges | Quickly identify patients with pressing needs that require follow-up. |
| Challenges Resolved | Track how many issues your team has successfully addressed. |

2. Patients: Manage and Track Care
The Patients tab lets you manage all patients assigned to you or your team.
| My Patients | View details for patients assigned to you. |
| All Assigned Patients | See your team’s full patient list to coordinate care. |
| Unassigned Patients | Identify patients who need to be assigned for follow-up. |
| Select User | Filter patients by team member. |
| Discharged Patients | See patients who have been discharged. |
To add a new patient, search by name or date of birth to ensure they’re not already in the system. Then click + Add New Patient and enter their details.
3. Reporting: Measure Impact and Generate Insights
The Reporting section gives you a clear view of your team’s impact and activity. You can view trends through interactive Graphs or generate detailed Reports for download and analysis. The graphs and reports that are present are based on your team's specifications and unique needs.
Graphs
The Graphs tab displays visual summaries of key program metrics over time. Many programs display the following metrics:
| Total Patients | View how many patients were added, assigned, or unassigned each month. |
| Patients Screened | Track how many patients have been screened at least once. |
| Screener Outcomes | See how many patients had at least one challenge identified versus those with no reported challenges. |
You can adjust the Start date and End date filters at the top to customize the time range and download your data as a CSV file for further review.
Reports
The Reports tab allows you to generate, view, and download detailed reports on your team’s work.
| Generate Report | Click Generate report, select the report type and choose a date range. |
| Frequency | Reports can be generated daily, weekly, monthly, or at a custom interval. |
| Actions | Use the eye icon to preview or the download icon to export a report. |
