Example: WIH Weekly Challenges and Solutions Count Report
Background
Women and Infants’ Hospital wanted a comprehensive weekly report that quantifies the number of new patients enrolled in their program stratified by medical provider, and a breakdown of all challenges generated and solutions executed.
Custom Report
Request
From a reporting standpoint, WIH requested:
- A WEEKLY report with 4 tabs
- Tab 1 (Patients Enrolled): Total # patients enrolled (1 column) stratified by Medical Provider (2 columns).
- Tab 2 (Patient Challenge): Numerous columns:
- Column 1: Total Challenges Created
- Column 2: Total Challenges Resolved
- Column 3: Total Challenges Created > 3 months ago
- Additional columns then used these same three columns for each INDIVIDUAL challenge in order to see data by individual challenges
- Tab 3 (Patient Solution): Numerous columns:
- Column 1: Total solutions executed
- Remaining columns: each individual solution executed with the total number under it
- Tab 4 (Patient Insecurity): Number of patients who answered a specific question in a specific way. In this case, WIH wanted to quantify housing insecurity, transportation insecurity, food insecurity, etc. For each social need, WIH provided the exact question in the screener along with the associated answers (e.g. Over the past 6 months, have you worried about having enough money for food? Y/N).
Outcome
Based on the above request, we generated a Custom Report with four tabs.
