How to request a Custom Report

We are committed to helping you meet all of your reporting needs. Our default automated reports cover common use cases, but we are also able to build reports tailored to more specific questions.

How the request process works

  1. Please fill out the Custom Report Request form. This form captures all of the information we need to build your report.
  2. We will review your request and will reach back out with follow-up questions, if needed.
  3. Before we begin building your report, we will also reach out to give examples of what your report will look like for approval.
  4. Once the report is built, you will be able to access it in the Reporting section of the application.

Example #1: CCF Project Undercover Report

Background

Connecting for Children and Families runs an internal program called CCF Project Undercover. When an individual accesses CCF Project Undercover, a CCF staff member clicks on the solution (“Referral”). When “Complete” is clicked on the solution, this solution is logged as executed in the data.

Custom Report

Request

From a reporting standpoint, CCF requested:

  1. A MONTHLY report
  2. Total # referrals completed that month (1 column)
  3. # patients who identify with the following (5 columns)
    1. Race White
    2. Race Black/African American
    3. Race Native American (American Indian)
    4. Race Asian
    5. Ethnicity Hispanic/Latino

Outcome

Based on the above request, we created an Excel report that is automatically generated monthly and also available to be run on-demand.

Example #2: CCF Marketplace Report

Background

Connecting for Children and Families runs an internal program called CCF Marketplace. When an individual identifies as food insecure, a challenge called “Food Resources Needed” emerges. This challenge has a number of associated solutions, one of which is: “CCF Marketplace”.

When an individual accesses the CCF Marketplace, a CCF staff member clicks on the solution (“Referral”). When “Complete” is clicked on the solution, this solution is logged as executed in the data.

Request

From a reporting standpoint, CCF wanted to track a number of fields as it relates to this specific solution.

For the CCF Marketplace solution, CCF requested:

  1. A MONTHLY report
  2. The associated note text
  3. Client Name
  4. Household Size of that client
  5. Age of each household member
  6. Number of total visits that month
  7. Whether it was the client’s first time to the CCF Marketplace

Outcome

Based on the above request, we created the following Excel report which is automatically generated on a monthly basis and also available on-demand.

Example #3: WIH Weekly Challenges and Solutions Count Report

Background

Women and Infants’ Hospital wanted a comprehensive weekly report that quantifies the number of new patients enrolled in their program stratified by medical provider, and a breakdown of all challenges generated and solutions executed.

Request

From a reporting standpoint, WIH requested: 

  1. A WEEKLY report with 4 tabs
  2. Tab 1 (Patients Enrolled): Total # patients enrolled (1 column) stratified by Medical Provider (2 columns).
  3. Tab 2 (Patient Challenge): Numerous columns:
    1. Column 1: Total Challenges Created
    2. Column 2: Total Challenges Resolved
    3. Column 3: Total Challenges Created > 3 months ago
    4. Additional columns then used these same three columns for each INDIVIDUAL challenge in order to see data by individual challenges
  4. Tab 3 (Patient Solution): Numerous columns:
    1. Column 1: Total solutions executed
    2. Remaining columns: each individual solution executed with the total number under it
  5. Tab 4 (Patient Insecurity): Number of patients who answered a specific question in a specific way. In this case, WIH wanted to quantify housing insecurity, transportation insecurity, food insecurity, etc. For each social need, WIH provided the exact question in the screener along with the associated answers (e.g. Over the past 6 months, have you worried about having enough money for food? Y/N).

Outcome

Based on the above request, we generated a Custom Report with four tabs.
