Setting up your account
Welcome to ITO HealthBase! Before you are able to start using the service, you will need to finish setting up your account.
You can set up your account in one of two ways:
If you received an email from ITO Health:
Click the activation link in the email. You’ll be taken to a page with instructions to complete your setup. Follow the on-screen steps.
If you did not receive an email:
Go to your organization’s ITO Health site — organizationname.itohealth.org — select “Create an account,” and follow the on-screen steps.
During setup, you’ll be asked to select the position that best describes your role (for example: case manager, social worker, or community health worker).
Don’t worry if your exact title isn’t listed — this won’t affect your access or available features.

| Email address | We use your email address to identify your account, to send important account notifications, and to verify your identity when accessing sensitive data. |
| Mobile phone number | We use your phone number to verify your identity when accessing sensitive data. |
| Lock code | After a period of inactivity, your screen will be locked. This helps protect your data. You can also manually lock your screen when you are stepping away. |
Logging in for the first time
Step 1: Go to your organization’s site: https://organizationname.itohealth.org
Step 2: Enter your email address and password.
Step 3: Enter the two-factor authentication code sent to your email.
Step 4: On your first login, you’ll be prompted to create a unique 6-digit lock code.
For security, your account will automatically lock after 5 minutes of inactivity. You’ll need your lock code to unlock it and continue your session.
