Editing/Correcting a Completed Screener
Screeners in ITO Health help identify patient needs and generate relevant challenges. Because challenges are based on responses in the Social Determinants of Health (SDoH) screener, you may occasionally notice a challenge that no longer applies or does not accurately reflect your patient’s current situation. This article explains how to correct or update a completed screener so that the patient’s challenges are accurate and up to date.
Reasons to make a correction
You may need to edit a completed screener if:
- A challenge appears that does not apply to the patient
- A patient’s situation has changed since the screener was completed
- A question was answered incorrectly during the initial screening
- A different challenge within the same category should apply instead
Before making changes, take note of the category associated with the challenge (for example: Food, Housing, Vital Documents). This will help you locate the correct section of the screener to update.
Example Scenario
Say that you're reviewing a patient’s Challenges page and see a challenge listed for “Eviction Legal Assistance.” After speaking with the patient, you realize this no longer applies—they are no longer a renter and are instead a homeowner seeking mortgage assistance. In this case, the goal is to update the screener so the correct housing-related challenge appears.

How to edit a completed screener
- From the patient’s profile, navigate to the Social Determinants Screening tab
- In the Steps sidebar on the left, locate the category associated with the challenge
- In this example, select Housing
- Select Edit answers for that category
- Update the screening responses to reflect the patient’s current situation
- Save your changes
Once updated, the system will reassess the patient’s responses and update the Challenges list accordingly.
