Viewing & Editing Patient Information

Want to review your patient's demographic information? Has your patient experienced a change in living situation or income? Would you like to add a new external case manager for your patient?

Overview

Patient demographic information helps ensure that screenings, forms, and referrals are accurate and up to date. You may need to update a patient’s information if their contact details change, their household situation is updated, or you notice an error in their profile.

Demographic Information

Demographic information includes identifying and background details about a patient, such as:

  • Name and date of birth
  • Contact information (phone number, email, address)
  • Income information
  • Household members and household size
  • Housing information
  • External case manager details

This information is used across the platform to support screeners, forms, and referrals.

Viewing & editing patient information

  1. Navigate to the Patients section
  2. Select My Patients
  3. Click on the patient you'd like to edit.
  4. Click on the Edit button next to the patient's name. You will also be able to view details here.
  5. Use the sections on the left to update demographic details. You can, among other things:
    1. Add household members, update head of household, switch order of household members
    2. Add additional contacts
    3. Add a new external case manager
    4. Add new sources of insurance
  6. Save your changes before leaving the page

Things to keep in mind

  • Some fields may be required and cannot be left blank.
  • Updating demographic information may affect screenings, eligibility results, or forms that rely on this data.
  • Only users with the appropriate permissions can edit patient information.
  • Be sure to save your changes before navigating away from the page to avoid losing updates.
