Completing the Social Determinants Screening
Overview
The Screener is a core tool within a participant’s chart. It provides a standardized set of questions across multiple social needs domains and uses smart branching logic to guide you through the most relevant questions for each individual.
The screener helps your team:
- Ask a consistent, structured set of questions across all participants.
- Identify social needs and determine eligibility for programs by embedding real eligibility criteria into the question flow.
- Automatically generate challenges and potential solutions based on participant responses.
You can access the Screener by clicking the Screening tab in the participant’s chart.
How the Screener is Structured
The Screener is organized into domains listed on the left side of the screen (e.g., Primary and Mental Health Care, Transportation, Financial Health, Utilities, Housing).
Each domain contains questions that may branch into follow-up questions based on how the participant responds. This branching logic is collaboratively developed between ITO Health and your organization.
Domain Progress Indicators
Each domain displays a color indicator showing completion status:
- Green: Fully completed
- Orange: Partially completed
- Gray: Not yet started
You can complete the screener in any order and move between domains freely.
Completing Questions
As you move through each domain:
- Select the appropriate responses for the participant.
- Additional follow-up questions will appear only when relevant.
- You can complete as much or as little as needed during a session.
You may click Save and exit at any time—your current responses will be saved immediately.
Automatic Daily Save
In addition to manual saving, the system automatically saves the current version of the screener every night at midnight, ensuring that your most up-to-date responses are always captured.
Viewing Past Versions
A Versions button is available at the top of the screener. Clicking this allows you to view previous versions of the screener that were saved in the past.
- Versions are sorted and filtered by date,
- You can open any past version to review how a participant’s responses have changed over time.
This feature is particularly useful for audits, longitudinal tracking, and understanding evolving participant needs.
Branching Logic
The Screener uses branching logic to streamline the process and ensure participants are only asked relevant questions. Examples include:
- Income questions appearing only when needed for eligibility assessment.
- Additional transportation questions surfacing when someone reports difficulty traveling.
- Utilities or insurance follow-up questions showing when responses indicate a risk.
This keeps the screener focused, efficient, and customized to the individual.
What Happens After the Screener
Once the Screener is completed—fully or partially—the system automatically:
- Generates Challenges based on identified needs
- Recommends Solutions aligned with relevant programs and actions
These appear in the Challenges tab, enabling a seamless transition from screening to intervention.
